Your initial appointment will be a consultation to discuss your diagnosis and treatment options. In some cases, treatment may be completed on the same day as your consultation.

Payment is due at the time services are rendered. If you need to reschedule an appointment, please give us at least 48 hours' notice to avoid cancellation fees. We accept cash, checks, Visa, and MasterCard. If necessary, we can arrange financial plans.

As a courtesy, we will submit your insurance claim on your behalf, provided you supply accurate and complete information. Please remember that your insurance is a contract between you, your employer, and your insurance company, and we are not a party to that contract. Consequently, you are fully responsible for service fees.
Any pre-estimate from your insurance is not a guarantee of coverage. We strive to help you maximize your benefits, but coverage depends on your yearly maximum, deductible, and eligibility at the time of service. If you would like to know your expected coverage, we can submit a pre-treatment estimate to your insurance.
Co-payments are due at the time of treatment. Please inform us of any changes affecting your billing or physical conditions, such as employment, address, new medical or dental issues, or insurance coverage.
Full payment is expected at the time of service for all patients, including those paying out-of-pocket. We accept cash, check, and credit cards (Visa, MasterCard).
